Implementing Effective Care Coordination Models for Advanced Illness Patients at Freeman Auditorium

This Regional Research Forum highlights three pioneering research studies funded by the Patient Centered Outcomes Research Institute (PCORI). The forum purpose is to place these actionable PCORI studies in the hands of implementers - health system leaders, individual providers, payers, community and state coalitions, and advocacy groups - who can use the research findings to improve the delivery of care for patients living with advanced illness.

The following PCORI-funded investigators will present their research:

  • Thomas Caprio, MD, Associate Professor of Medicine / Public Health Sciences / Nursing, University of Rochester School of Medicine
  • Sarah Scholle, DrPH, VP of Research and Analysis, National Committee for Quality Assurance
  • Nick Faris, Director of Thoracic Oncology at the Baptist Cancer Center, Memphis, TN

The forum is convened by the Coalition to Transform Advanced Care as part of a PCORI Eugene Washington Engagement for Capacity Building Award. The sponsor is The UTHSC Center for Health System Improvement, which is currently conducting the PCORI-funded Management of Diabetes in Everyday Life (MODEL) study.
